The level of care we provide will vary according to the individual’s needs and potential. Elements to be provided by the Service will include, but will not be limited to:

- Supported Living Accommodation and support in own home or Shared Accommodation.

- Semi- Independent Accommodation and Support.

- Home From Hospital- We deliver services to support people on discharge from hospital. The Home from Hospital service will enable people to be safely discharged from hospital or other Health or Social Care setting at the earliest opportunity to a home setting and will help people re-adjust to living at home again, gain in confidence, and provide short term practical help after hospital discharge. The length of the support depends on individual circumstances.

- Live-in- 24- hour Care and support- is suitable for a wide range of health and social care needs including personal care, meal preparation, managing and maintaining nutrition, support to maintaining a habitable home environment.

- Medication administration and monitoring..

- Dementia care- is provided by experts and competent staff who has experience and understanding of the condition.

- Elderly care- is for people in later life and who need a bit of extra support with daily living.

- 24-hour care- is for people with intense needs who require support every hour of the day.

- Stroke care- is for stroke or accident survivors who need support with rehabilitation and living well.

- Home help- is companionship care for those who want a friendly face around to support them.

- Support to make use of necessary facilities or services in the local community engage in community and reduce isolation.

- Encouragement and support to develop and maintain family or other personal relationships.

- Promotion of health and wellbeing.

- Multidisciplinary and integrated working.

- Conducting risk assessments and managing risks.

- Adherence to safeguarding procedures .

- Support to relatives and Carers.

- Infection control procedures.

- Palliative and end of life care .

Referrals and How to access the Service.

Connections24Hrs Limited will have a face to face meeting with the Service User before the start of the Service in order to produce a person-centred care plan, detailing how their outcomes will be achieved and how their support will be delivered, except in emergency situations when this must be done within the first 48 hours. This meeting may involve the Service User’s family carer or advocate. At Connections we provide Supported Living Accommodation and Mentoring Support to Looked after Children and Care Leavers.

Our various Supported Living / Lodging packages are tailor made to cater for high risk individuals needing to develop and improve their independent living skills and/or Asylum Seekers. Connections is also a Nursing Agency providing agency nurses to mental health hospitals.

Connections24Hrs is a regional provider of temporary employment solutions for nurses and support workers that work in Nursing Homes, Residential Homes, NHS Hospitals, Private Hospitals and Homecare. Our nursing and healthcare staff benefit from the extensive range of clients we supply and our clients benefit from receiving top quality, experienced, reliable staff. Our staff are all DBS registered and they are provided with regular supervision.
